RSVP form option 2 – Food choices

This is for a sit-down dinner where attendees needed to submit their meal choices ahead of time. (It is my winter party 2019 with food choices). This allows for 2 people but the “conditional” can be created to allow for more. There can be as many or as few food choices. I had an Earlybird pricing form and a Regular priced form, changing one for the other at the deadline.
